GLP-1 prices are moving. The terms matter more than the headline.

What changed

Lilly currently lists Zepbound KwikPen self-pay from $299 per one-month fill. Some other purchase offers depend on a refill within 45 days. NovoCare lists an introductory $199 per month for two eligible new-patient Wegovy pen fills through December 31, 2026, followed by $349 per month for listed standard pen options. These are manufacturer self-pay patient offers, not wholesale or clinic acquisition prices, and eligibility and terms apply. [1][2]

Why the pharmacy path matters

FDA says compounded GLP-1 drugs are not FDA-approved and do not undergo the agency's premarket review for safety, effectiveness and quality. In its April 2026 update, FDA said semaglutide and tirzepatide were not on its shortage list or 503B bulks list. It also described different restrictions for patient-specific 503A compounding and 503B outsourcing. A low quote by itself cannot tell a clinic whether an offer is appropriate or compliant. [3][4]

Demand meets price sensitivity

In a November 2025 KFF poll, 12% of US adults said they were using a GLP-1 drug, while 56% of users said the drugs were difficult to afford. That national survey is not a forecast for any one practice. It does suggest that the full patient price, not just the first-month figure, deserves a clear answer. [5]

What exactly is included in this quote, and what changes by dose or refill?

What it means for a clinic: Ask a supplier to identify the product and formulation, quantity, actual patient or clinic price, refill conditions, shipping and supplies, and the pharmacy's licensure. Compare like with like. Do not share patient data in a first business inquiry.

Clinic Owner Brief is published by Clinic Wiz, which helps clinics compare options through pharmacy partners. A Clinic Wiz price check carries no service fee and does not obligate a clinic to change suppliers.
